Traditionally, they are tied around the wrist until Noon on July 6th when they are officially tied around the neck during the Txupinazo Opening Ceremonies. If you plan to run with the bulls it is highly advised that you tie the Pañuelo with a slip knot to avoid a bull horn catching it during the run.
